What few times he does have it they ususally show it sticking out. The other times he's wearing a uniform or something else so he can conceal it or holster it. And the rest of the time he usually doesn't have a gun, because he's either captured or in a situation where he doesn't need a gun. Atleast that's what I've always thought. I don't think the creators really got that much into detail about it unless he needed to use the gun...

My theory is that there's some kind of alternate univerise inside Heero's pants which allows him to conceal his gun without it showing. He then takes it out when called for.

This would also explain where Wing Zero's buster rifle was at the first part of Endless Waltz. Wing Zero just whipped it out of his magic glutious maximus.
