First, the Blinding Flash of the Obvious
Tsukasa's "animus" is actually _in_ the game.
Other thoughts.
_Something_ apparently connects Tsukasa to his/her body, despite not wearing a headset. While his/her "self" is in the world, things in either still affect one another. Also, Tsubasa somehow gets back inside his/her body in the end.
Something is sustaining Tsukasa's body. It's implied that it's not respirators or feeding tubes, though. One doctor was noting that Tsukasa's body wasn't accepting nutrients.
However, Tsukasa was in The World for 6 MONTHS! Not accepting nutrients for that long should be fatal!
Upon waking up (if you don't follow the "Morgana's tricked them into thinking they've waked up" theory) Tsukasa is able to easily walk out of the hospital. Hospital policy aside, muscles of coma patients for a long period of time should of atrophied some and require physical therapy.
Subaru, while in the game, sometimes shows flashes of a psychic link with Tsukasa. When he gets hurt particularly badly, the view will change to her being startled. (I'm not sure, but a single feather falling to the ground seems to be symbolic of whenever she's doing this.)
She also has the uncanny ability later in the series to zero in on Tsukasa and know wherever Tsukasa is. (And likewise Tsukasa with her.) They've made comments on simply "knowing" that the other would be there. Seems to be some relation between this bond and fireflies.
In the Real World (if you don't follow the "Morgana tricked them" theory) they both recognize each other despite not resembling their online persona's physically.
