I think Tetsuo went back to the Earth like in the Gaia myth (http://www.lawrence.edu/dept/environmental_studies/gaia.html). It's pretty simple to understand if you see it that way.

I think he ate some bad shellfish and had an allergic reaction. That would explain the swelling.
